Anticoagulant Service

General Information

 

The Anticoagulant Service currently provides services for approximately 2000 patients requiring anticoagulant therapy for specific medical conditions e.g. Deep Vein Thrombosis, Pulmonary Embolism, Atrial fibrillation (AF) for a given time or lifelong condition.

 

The Service is led by Consultant haematologist Dr Leila Khalil assisted by the Anticoagulant Coordinator (a specialist Biomedical scientific officer) J. Aistrup, with all national standards for anticoagulation services adhered to.

The Anticoagulant Service will:-

  • Provide assessment and counselling sessions.
  •  Provide short and long term follow-up care.
  • Initiate Anticoagulant therapy.
  • Provide advice and information for patients regarding their treatment.
  • Monitor patients in order to maintain them within their given therapeutic range.
  • Deal with any other issues relevant to the patient’s anticoagulation therapy.
  • Provide ongoing support, primarily via the telephone, for patients, relatives and carers.
  • Provide information and advice to other healthcare professionals
  • Provide International Normalised Ratio (INR) testing.
  • Provide advice on VTE risk assessment and thromboprophylaxis.

 

New patients referred to the Anticoagulant Service are primarily assessed in an outpatient clinic held within the hospital. 

Anticoagulant clinic timetable (excluding Bank holidays).

Monday

09:00-10:00

Friday

14:00-15:00 (ward discharges only)

 

Once assessed for suitability patients will be transferred to a postal service in order to provide them with more flexibility. They can attend a local blood test centre of their choice Monday to Thursday. The bloods are tested in the Haematology department and checked by the anticoagulant team. The results, dosage and next appointment are then posted back. Patients are contacted directly by the anticoagulant team if there are any urgent issues relating to their treatment.

How to register patients with the Anticoagulant Service

To arrange an appointment with the Anticoagulant Service, patients must be referred by a Hospital, Consultant or GP.
